Things You May Need To Know
When Planning A Funeral

Death Certificate
To complete the Certificate of Death, the family shall provide the following information:

full name of decedent; place of death; usual residence; date of death; sex; race; highest level of education; date of birth; age; birthplace (State only); citizenship (Which Country); usual occupation and industry; veteran status; social security number; father's name; mother's maiden name; marital status and name of spouse, if married or widowed; informant's name, doctor’s name which will sign the death certificate

Military Information
If military honors and benefits are desired, honorable discharge papers or DD214 discharge papers are requested. If these are not available, the funeral home will diligently attempt to get authorization.

Special Instructions
For the arrangement conference,
